What will Dental Blue ® cost? With Dental Blue ®, you don't pay a deductible for
checkups or diagnostic and preventive services. You're also covered for both basic services like routine fillings and extractions, and major services including crowns, bridges and dentures. All that's
required is a $ 75.00 annual deductible and 50% coinsurance.

You can choose Dental Blue® coverage
for you, your child or your entire family. There are three payment options: monthly bank draft, semi-annual payment or annual payment.

You don't have a waiting period to receive preventive
services with Dental Blue ®. Checkups and cleanings are covered twice per benefit period. So, you can protect your teeth by visiting the dentist every six months.
